I went through all of the SW beta's tiles with ART2BMP by Samiel and a program called DupFinder to find all the unique tiles in the SW beta.
There are some lost enemies that are referenced in Keith Schuler's list of sound entries such as Lava Boss, Skeletor Priest, and more. I found some art for at least four lost enemies, although Lava Boss only has one frame.

This one appears to be from very early screenshots of Shadow Warrior 3D.

The second one must be VERY old, since it has an Apogee logo, some very undeveloped art, and a part of EDITART.EXE on the bottom. It is actually two tiles, but I juxtaposed them in the HTML.
3D Realms screen:
The Duke 3D Atomic version is used. On the left is the raw art, on the right is with the correct palette.
Throughout the art, many copies of this Tile #are scattered, where some of the art was sadly removed.
There are even copies of it superimposed on top of other art! Argh!

The term "Lava Boss" is referenced both in Keith Schuler's list of sound entries as well as a file in the SW source code called lava.c.
MrFlibble alerted me to the fact that the Lava Boss appears as a cameo in the final game in a poster, under the name of "Earth-Eater" (Tile ##5049).

There is a nearly useless file in the SW source code called eel.c, which contains some code for a lost enemy. It can be inferred that the following lost enemy is the Eel. Note that the art is incompletely assembled, as the artists accidentally left the 320x200 editart box around a few of the tiles. In addition, the animations are in the wrong order to be used by the game.

The tiles for this are still in the game, but unused in the final. A TC called "The Last Warrior" used them. In 2008, ProAsm added an option to use it to his awesome SWP port.
